Authorities are seeking to prosecute over the 2011 car crash which claimed the lives of prominent architects, from Troppo Architects and Taylor Cullity Lethlean.

Federal Government workplace safety authority Comcare has announced intentions to prosecute Airservices Australia over the crash in Darwin. Troppo Architects directors Lena Yali and Greg McNamara, and principal of Taylor Cullity Lethlean landscape architects, Kevin Taylor, died after their vehicle was struck by a fire truck, operated by Airservices Australia, which went through a red light.

According to reports by the ABC, a coroner was scathing of the events that led to the incident. An inquest into the deaths highlighting major shortcoming “in the policies, operating procedures and training protocols of Airservices Australia”.

Coroner Greg Cavanagh had said: "It appears to me that since their main business is to operate at the airport, staff were ill-equipped to drive the ultra large fire vehicle in emergency conditions, at speed, with lights and sirens, on a public road."

A statement from Airservices Australia acknowledged the likely legal action, adding that the organisation was “well advanced in addressing recommendations from the 2013 coronial inquest into the crash”.

In 2012 the family of Ms Yali publicly requested that the firetruck driver not be charged over the crash, and charges initially made were later dropped by the Director of Public Prosecutions.
